Once the request is submitted, our staff will promptly get back with you to establish an appointment. For patients without insurance, please gather household income information to have available when our staff contacts you for an appointment. This includes the number of people that live in your household as well as total income for the household, including but not limited to:
- Employment income
- Social Security Benefits
- Food Stamps
- Disability
- Child Support Received, etc.
Having this information available will make your registration smooth and easy, allowing us to treat you as soon as possible.
